Sumter County Schools Families,

We're excited to share a partnership with Daybreak Health. Daybreak Health provides access to care, after-school hours, evenings and weekends, mental health care for your child. With parental consent, your child can receive 24 sessions of teletherapy. The most common reasons students seek support are related to school stress, anxiety, depression, peer to peer relationships, trauma, stress, disordered eating, and other common mental health conditions.

To schedule an appointment time after school hours (between the hours of 4-8pm EST or on the weekends, please visit this parent-facing video that explains how to fill out the referral form and schedule the appointments after 4 PM EST. You can also click on this link to create a Care Request for your Child.

For younger students (ages 5-9) Daybreak offers a family therapy program where you attend the sessions with your child. For older students (ages 10+) Daybreak offers a 1:1 program where your child meets individually with a clinician. For more information or to speak with a Care Coordinator email carecoordinator@daybreakhealth.com or call 415-992-6155

Thank you,
